Making an appointment

​

• You must first be referred to our practice by another provider and must have already completed an imaging study (CT, MRI) within  a year prior to the initial consultation.

 

• Once all necessary information is obtained from your referring provider, our scheduler will contact you to go over our patient questionnaire.  Please be prepared to answer questions pertaining to your condition, insurance and demographic information.

 

• Once an appointment is booked, please keep in mind that our physicians provide “on call” service to John Muir and your appointment may be rescheduled if any emergency surgeries arise.  If this happens, we will inform you as soon as possible.  

 

• Download the new patient registration forms here and have it completed prior to your appointment.  We can also mail the registration forms to you upon request.

 

• If you are unable to complete the forms prior to your appointment, you must arrive at the office at least 45 minutes early to fill them out.
