• You must first be referred to our practice by another provider and must have already completed an imaging study (CT, MRI) within  a year prior to the initial consultation. If your imaging was not done at John Muir Walnut Creek, Concord or Brentwood, you will need to take a CD with the images and report to one of these locations to have it transferred to the John Muir Radiology system.  We will not book appointments until an imaging study has been reviewed by one of our providers.

 

• Once a referral is received our medical assistant will call you to ask a few questions pertaining to your condition, insurance and demographic information.  We will also verify your imaging study is in the John Muir Radiology system.  Feel free to reach out if you have not heard from us within a few days.  We prefer the telephone for most communications.

 

• Once an appointment is booked, please keep in mind that our physicians provide “on call” service to John Muir and your appointment may be rescheduled if any emergency surgeries arise.  If this happens, we will inform you as soon as possible.  

 

• Download the new patient registration forms here and have it completed prior to your appointment.  We can also mail the registration forms to you upon request. Please bring your insurance card and ID to your appointment.

 

• If you are unable to complete the forms prior to your appointment, you must arrive at the office at least 45 minutes early to fill them out.
   

Co-payments are due at the time of service.  We accept Visa, MasterCard, Discover and debit cards.  We do not take cash, checks or American Express.
